Important information for filling out the raffle participant’s questionnaire:
1. If the amount of your check is a multiple of the initial participation amount (1500 UAH), you must register all your chances. For example, if the amount of your check is UAH 15,000, you have 10 chances to win the Grand Prize. For this you need to register your check 10 times.
2. The fiscal code of the check is indicated in it as FN. In the application form, only the numbers of the FN should be indicated.
3. The photo of your check must clearly indicate the place (address) of the purchase, the number of the fiscal check, the date and amount of the purchase.
4. If a mistake is made or if some information is not specified in the questionnaire, our manager will contact you.
